This notice describes how medical information about you may be used and disclosed and how you can get access to this information. It also advises you of your rights regarding your medical information. Please review this information carefully.
Grogan's Healthcare Supply has always respected the privacy of our customers and their healthcare information. This notice describes how we may use and disclose protected health information about you. Protected health information is any health information about you that identifies you or for which there is a reasonable basis to believe the information can be used to identify you.
How we may use and disclose medical information about you:
1. For Payment
We may use and disclose health information about you so that we may receive payment for the services or products we provide you.
2. For Our Own Internal Operations
It may be necessary for us to discuss your medical information internally, with appropriate staff, in order to make determinations about the products or services we are providing to you or to file claims on your behalf.
3. To Individuals Involved In Your Care
Unless you object, we may disclose to a family member, other relatives, a close personal friend, or any other person identified by you, medical information about you that is directly relevant to that person's involvement with your care or payment for services or products we provide.
4. As Required By Law
We may disclose medical information about you when we are required to do so by law. This may include reporting to public health officials, government agencies or other legal authorities.
5. Other Uses and Disclosures
Other uses and disclosures will only be made with your written authorization. You may revoke such an authorization by notifying in writing Grogan's Privacy Officer
Your rights with regard to medical information about you:
1. Right to Inspect and Copy
You have the right to inspect and copy medical information about you, at your expense. To request a copy of medical information about you or to inspect medical information about you, you must submit your request in writing to Grogan's Privacy Officer.
Your request should state specifically what information you want to inspect or have copied. We will respond to all requests within 30 calendar days after we receive your written request. We may deny your request to inspect or copy medical information. If we deny your request, we will inform you of the basis for our denial, how you may request a review of our denial, and how you may complain.
2. Right to Receive Confidential Communications
You have the right to request that we communicate medical information about you to you in a certain way or to a certain place. If you want to direct such communications please submit your request in writing to Grogan's Privacy Officer.
3. Right to Request Restrictions
You have the right to request that we restrict the uses and disclosures of medical information about you. To request restrictions, please submit in writing what information you want to limit, whether you want to limit use or disclosure or both, and to whom you want the limits to apply. Submit requests to Grogan's Privacy Officer.
4. Right to Amend
You have the right to ask us to amend medical information about you to correct incomplete or incorrect information. You have this right for as long as we maintain medical information about you.
5. Right to an Accounting of Disclosures
You have the right to receive an accounting of our disclosures of medical information about you. The accounting may have been made up to six (6) years prior to the date on which you request the accounting but not before April 14, 2003. Certain types of disclosures are not included in such an accounting.
To request an accounting of disclosures you must submit your request in writing to Grogan's Privacy Officer. We will generally respond to such requests within 60 days of receipt of your request. If there will be a charge for the copying of the disclosure or the cost of mailing we will notify you in advance.
6. Right to a Copy of this Notice
You have the right to obtain a paper copy of our Notice of Privacy Practices. You may request a copy by writing Grogan's Privacy Officer.
Our Duties and Responsibilities
We are required by law to maintain the privacy of medical information about you and to provide individuals with notice of our legal duties and privacy practices. We are required to abide by the terms of our Notice of Privacy Practices.
